I have created a custom Language (Swedish) file with name sv.xml (from no.xml)

In the Configuration panel i set:

Culture Type =  Server

Custom Culture = sv 

But the language is on english?

Please restart your IIS or your site after this language setting. If still does not work, then please post the full code of your new sv.xml file. So we can check it for you.
