I think I have determined the source of the problem.  The web server required NT authentication.  I set this on the page that opens the editor.  However, apparently, this does not carry over to the process that creates the PDF.  The images are linked content that requires NT credentials and the process just leaves them out since it does not have access to use them.  I temporarily changes the development server to allow anonymous access and the PDFs are created fine.  So, now I just have to figure out how to handle it with NT authentication in place...
