I am very appreciative of the speed of response and your attention. It seems that I should be seeing more icons on the Insert Pages. I only see the "To Parent" and "New Folder" icons.

 

As I mentioned originally, I think they may be missing from my system. Can you please tell what would cause this and where I should look to discover what is going on?
