However, the issue is not in the deployment.  It is within the Visual Studio form itself.

 

This form has the exact code supplied in the form control supplied by CuteSoft ("Classic-Layout.aspx").  However, there is an error message attached to the control in the form indicating that I am using an invalid license file and the property "GalleryClient" indicates the error "Object not set to an instance...".

 

Further, when I run the site under the VS Development Server enviornment in debug mode, I get a a number of js errors.
